All-on-Four Implants

All-on-Four is a technique that secures a full arch of teeth onto just four titanium implants. The implants are placed in the strongest areas of the jaw, and because the two rear implants are angled, most cases need no additional procedures such as bone grafting. You leave the clinic with fixed temporary teeth on the day of surgery. In Istanbul the treatment takes four days, within a five-night programme.

What is All-on-Four?

All-on-Four is an implant technique developed for people who have lost all or most of their teeth. Instead of placing a separate implant for every tooth, a fixed bridge carrying the whole arch is screwed onto just four implants.

What sets the technique apart is the angle of the implants: the two front implants are placed straight (90°), while the two rear implants are positioned at 30–45°. This angled placement avoids the sinus cavities and nerve canals, and because it makes use of the denser parts of the existing bone, most cases need no bone graft.

Advantages

  • Only 4 implants are needed for a full arch
  • In most cases no bone graft or sinus lift is required
  • You leave with fixed temporary teeth on the day of surgery
  • A fixed solution, not a removable denture
  • Chewing function and facial support are restored

Temporary and permanent bridges

Once the implants are placed, a temporary hybrid bridge is screwed onto them the same day. This means you return home without being left without teeth, and go through the healing period with fixed teeth.

It takes around three months for the implants to fuse fully with the jawbone. After this period the temporary bridge is removed and your permanent bridge is made and fitted.

How does it work, step by step?

1

Online consultation (from home)

If you have a dental X-ray, you share it with your photos over WhatsApp. The dentist makes an initial assessment and we send whether All-on-Four is suitable for you, along with a personalised quote, within 24 hours.

2

Day 1 — Surgery and temporary teeth

After the examination and X-rays, four implants are placed into the jawbone under local anaesthetic. The procedure takes around 2 hours. The temporary hybrid bridge is screwed onto the implants the same day, so you leave the clinic with fixed teeth.

3

Day 3 — Try-in appointment

The fit, bite and appearance of the bridge are reviewed together. The day in between is free for you to rest and explore Istanbul.

4

Day 4 — Aesthetic work

The final aesthetic adjustments are made and the bridge is secured. Advice on care and diet is explained at this stage.

5

Day 5 — Final check-up

Your bite and gum fit are checked one last time. After this you are ready to fly home, and a short second visit is planned around three months later for the permanent bridge.

Who is it suitable for?

  • Adults who have lost all the teeth in a jaw
  • Those who do not want a removable denture
  • Cases where bone loss makes conventional implants difficult
  • Those wanting fixed teeth within a short time

When might it not be suitable?

  • Anyone with untreated gum disease
  • Some cases with very advanced bone loss
  • Uncontrolled systemic conditions such as diabetes
  • Heavy smoking — it can affect healing

Frequently asked questions

How long do I need to stay in Istanbul?
On the first visit the treatment takes 4 days within a total of 5 nights’ accommodation, with appointments on days 1, 3, 4 and 5. A short second visit is needed around three months later for the permanent bridge, and we organise both trips for you.
Will I be left without teeth after surgery?
No. A temporary hybrid bridge is screwed onto the implants on the day they are placed, so you leave the clinic with fixed teeth. You go through the healing period with these temporary teeth and receive your permanent bridge around three months later.
Why are four implants enough?
The implants are placed in the areas of the jaw with the highest bone density, and the two rear implants are angled at 30–45°. This angled placement spreads the load over a wider area, so a fixed bridge carrying a full arch can be supported on four implants.
Will I need a bone graft?
In most cases, no. The angled placement makes use of the denser parts of the existing bone and avoids the sinus cavities. In cases of very advanced bone loss an additional procedure may still be needed, which is assessed after X-rays and examination.
Is the procedure painful?
The procedure is carried out under local anaesthetic, so you should not feel pain during it. Swelling and tenderness for a few days afterwards are normal; your dentist can recommend pain relief and will advise soft foods in the early period.
